White Chapel Hospital and Maternity is a secondary health care center that provides healthcare to a population of over 100,000 patients.
I work on a rotational basis in the General Medicine and Surgery Department for six month in each department. Currently, I'm in the Medical department. My duties are highlighted below:
As part of the requirements for obtaining a full registration with the Medical and Dental Council of Nigeria, was required to undergo an internship (housemanship) under the supervision of registrars and consultants.

Project title: Causes of Mortality among Neonates of Diabetic Mothers.
Date: September 2020
Aim of research: To identify immediate causes of Neonatal mortality among Neonates of Diabetic Mothers at Chukwuemeka Odumegwu Ojukwu University Teaching Hospital, Amaku, Awka, Anambra State.
Conclusions/Actions: Hypoglycemia was the commonest cause of deaths in these neonates. Actions were made to ensure they have their blood sugar checked at birth and monitored.
1. A Quality Improvement on Clinical Documentation and Record Keeping at White Chapel Specialist Hospital and Maternity.
Aim & measure of the change: To improve the accuracy of medical records and the quality of clinical documentation by training clinicians on the use of computers and digital record systems.
Role/Conclusions/Actions: I made a proposal to the management to institute digital clinical record system. Following training and implementation, there was a significant difference in the efficiency of obtaining patient's medical records and clinical documentation.

2. Early Detection of Gestational Diabetes.
Aim & measure of the change: To prevent adverse pregnancy outcomes secondary to gestational diabetes by ensuring that pregnant women at risk are being screened in the unit.
Role: Conclusions/Actions: I encountered 2 cases of hyperglycemic emergencies in pregnancy that was due to failure to diagnose gestational diabetes. This was discussed in the clinical meeting and a 24-28 weeks OGTT screening was introduced. I was responsible for the screening of patients. A reaudit showed no further cases in the 6-9 months following the implementation of the screening.

3. Reduction in the rate of Nosocomial Pneumonia in the Surgical ward at Chukwuemeka Odumegwu Ojukwu University Teaching Hospital.
Aim & measure of the change: The aim was to assess and reduce the rate of hospital acquired Pneumonia in the Surgical wards. This was to be checked by keeping track of incidents of hospital acquired pneumonia.
Role/Conclusions/Actions: I was involved in creating awareness posters and educating ward staff about proper hand washing, use of alcohol based sanitizers and face masks, we found that there was a drastic reduction in the spread of infection in the surgical department and patients overall wellbeing improved.

Recently, I delivered a lecture on 'Management of Acute Abdomen' to a group of medical students and the feedback I got was very positive. Teaching is always a delight to me and I'm always happy to take up the role when called upon.
I have learnt to delegate and take charge when the situation arises. At the same time, I am very much aware of my limitations as a leader and do not fail to seek for support from my teammates and/or seniors.
