A government owned hospital, established in April 1983 with approximately 100-300 beds capacity and standard medical equipment.

Currently in medsurg unit with 25 beds capacity and a nurse-patient ratio of 1:5 or 1:6 in case of emergency situation which requires speed and coordination in other to provide urgent and unexpected patient care.

• Plan admission and discharge process.
• Conduct comprehensive initial head-to-toe assessment and ongoing reassessment tomonitor progress of patients and improve individual nursing care.
• Monitoring of vital signs inclusive of pain score,GCS,and NEWS.
• Provision of supplemental oxygen therapy via method that soothes patient's conditions and airway management.
• Administer prescribed medications via oral,I.V,I.M,SC,intradermal,topical,patches, Infusions and monitor responses.
• Assist patient with mobility and personal hygiene needs
• Monitor and document patient fluid intake and output and bowel movements.
• Plan,prioritize and Provide nursing care by applying advance nursing clinical knowledge and skills to ensure patients safety.
• Initiate life-saving Interventions e.g CPR,calling rapid response team
• Prioritize care for multiple patients with varying level of acuity.
• Adult male and female catheterization and care to prevent CAUTI
• Develop,review and implement individualized nursing care plans for optimal outcomes.
• Observe and record signs and symptoms,behaviors and physiological status of patients and notify physician as appropriate
• Provision of pre and post operative care using individualized nursing care plan an, monitor for complications.
• Manage wounds using aseptic technique and care of pressure areas.
• Monitoring of blood glucose level and glucose management .
• Daily weighing of patient .
• Siting peripheral vascular access device and collection of blood sample and other bedside specimen.
• Transfusion of blood and other blood products , Monitoring of patient before,during and after transfusion and intervention where necessary.
• Document nursing assessments interventions and outcomes In electron9c medical record (EMR) with data such as medication update,care provided ,interventions etc. to keep records current and support accurate treatment.
• Perform insertion of nasogastric tube,Airway devices,sunctioning.
• Provide care of central linestracheostomy tube,chest tubes,stoma and other drains and adhere to infection control protocols.
• Assist patient with activities of daily living.
• Collaborate with multidisciplinary teams to promptly assess patients and deliver appropriate treatment and ensuring holistic care is rendered.
• Promote patient and family comfort during admission
• Educate patient and relation on diagnosis, prognosis treatment plans,post-discharge care to enhance active participation in care and prevent non-compliance to treatment regimen.
• Accompanying patient for diagnostic investigation.
• Ensuring a safe environment by following infections control and safety protocols.
• Monitor,recognize and response to signs of clinical deterioration e.g sepsis,bleeding,respiratory distress, decreased level of consciousness and interven appropriately.
• Provide patient-centered care in alignment with hospital policies.
• Manage workload efficiently and delegate task to appropriate personnel when necessary according to hospital guideline.
