  • Functions as the Pharmacist on the Community ART Management (CAM) team of the ECEWS ACE 5 project in Akwa Ibom state.
  • Ensures observance of Pharmacy Best Practices ideals in assigned Community ART Management (CAM) team
  • Actively supports implementation of DDD models such as CPARP, CARC, CARG and DARF to enhance clients' access to drug refills, improve retention, viral suppression and improve clients' satisfaction.
  • Supports active linkage of clients identified by the CAM team to facility and other DDD models in the community.
  • Devolves eligible and willing clients to the various DDD models in the community and facility with the aim of de-congesting high volume sites, improving access to ARV refills, clients' retention, and viral suppression.
  • Supports Community Pharmacies to distribute commercial HIVST kits through a Total Market Approach (TMA) and the linkage of reactive clients to facilities for confirmatory testing and HIV care and support services.
  • Supports the initiation of all identified positive HIV clients who have been provided HIV education and adherence counselling and are ready, to commence life-long ART.
  • Monitors and screens all ART clients for drug therapy problems including Adverse Drug Reactions (ADRs), medication errors, medication non-adherence and provide appropriate interventions to resolve them.
  • Promotes clients’ retention in care across all facilities and community ART sites by profiling them to identify their unique adherence barriers and resolving them to keep them on treatment.
  • Ensures medication adherence and viral suppression of assigned clients in the community and facility through provision of quality optimal adherence counselling at ART initiation and during every drug refill visit.
  • Provides Enhanced Adherence Counselling (EAC) to unsuppressed clients in the community and facility using the necessary job aids and tools as well as monitor the clients throughout the EAC sessions.
  • Supports facility level Drug and Therapeutic Committee/Pharmacovigilance Committee (DTC/PVC) to discharge its responsibility of ensuring rational drug use to optimize drug therapy.
  • Takes responsibility for the provision of pharmaceutical care to assigned clients in the community and facility ART dispensing settings.
  • Responsible for effective and efficient inventory management of ARVs and OI medications used in CAM teams and hub facilities and, carry out weekly stock reconciliation between the hub and the CAM team.
