DESIGNATION: Registered Nurse

UNIT: Accident and emergency

• CASES HANDLED: Head Injury, Unconsciousness, Fracture, Hemorrhage, Road traffic accident, cardiac arrest, cerebral vascular accident, brain injury, food poisoning, drug/substance abuse, fall from height, electrocution, burns/ fire accident, drowning.

• ROLE AND RESPONSIBILITIES: Collect current symptom, as well as a detailed patient history and then consult and coordinate with health care team member to assess, plan, implement and evaluate patient care plan
• Performs triage, while considering both physical and psychosocial elements
• Manage basic life support need and stabilize patient until the attending physician is available, based upon nursing standard and protocol
• Initiate corrective action whenever information from monitoring equipment show adverse symptomatology
• Records patient’ current vital signs
• Works directly under physician, assisting them during examination, diagnostic testing and treatment
• Prepare and administer (orally, subcutaneously, intravenous and intramuscular) and record prescribed medication.
• Report adverse reactions to medication or treatment in accordance with the policy regarding the administration of medication by a licensed registered nurse
• Provide intravenous therapy
• Clean and bandage wound
• Blood and blood products administration
• Provide basic, bedside care
• Maintains proper supplies and appropriate medical equipment to care for patients
• Initiates patient education plan, as prescribed by physician.
• Teach patients and significant others how to manage their illness/injury, by explaining: post-treatment home care needs, diet/nutrition/exercise programs, self-administration of medication and rehabilitation, as well as provides referrals to other healthcare professionals for follow-up treatment
• Effective communication with clients and health care team.
• Assess, plan, implement evaluate nursing care and contribute to the efficient management of the ward/unit
• Managed patient promptly informing, informing physician of any vital changes in the status of patient and documenting information
• Proper monitoring of fluid intake and output, vital sign

• UNIT: Medical Surgical Ward
• CASES HANDLED: Managed various post-operative and medical-surgical cases involving female patients.

ROLES AND RESPONSIBILITIES

• Administered medications and treatments as prescribed by physicians.
• Coordinated with other healthcare professionals both within and outside of the facility to ensure continuity of care and appropriate resource utilization for patients requiring specialized services or treatment modalities.
• Admission and discharge of patient
• Preparation of a nursing care using the nursing process tool

• CASE HANDLE Taking deliveries (Normal and Complicated)
• ROLES AND RESPONSIBILITIES Close monitoring of patient from time of conception till postnatal stage
• Monitor contraction and the patient with breathing technique (using CTG medical)
• Perform basic IV and central line skill
• Administer pain medication as ordered, administer other medication to address some certain medical condition
• Take and record patient vitals, ascertain if problematic
• Assess fetal heart beat using doppler fetal monitor
• Identifies and manage life sustaining physiological function in unstable patients
• Cervical examination to ascertain dilatation and effacement
• Ensure that patient is in prepare position for delivery and all monitoring equipment are functioning properly
• Encourage patient to push at proper time
• Record time of birth and immediate care for new born
• Record Apgar score and keep patient relative informed on the progress of birth
