· Coordination of the Paediatric and Adolescent ART surge strategy (PASS) team for Kaduna towards achieving the three 95's
· Lead a MDT responsible for HIV program implementation across 40 sites
· Technical assistance to the facilities in an ongoing intensified HIV case finding drive through targeted PITC, family index testing (FIT), SNT amongst others.
· Collaborate with the HTS team in strengthening health system capacity to provide HTS services to all age group and other prevention services.
· Scale up uptake of prevention, care and treatment services at the community through collaboration with the Community-based organizations (CBOs) and NEPWHAN.
· Supportive supervision to 2 OSS in provision of prevention, care and treatment services to key populations including MSM, FSW and PWIDs.
· Liaise with the CBOs in identification and provision of preventive and supportive services to vulnerable groups including orphans and vulnerable children (CALHIV, Children of PLHIV, Children of KP, School drop-out, GBV survivors etc.), caregivers of CLHIV amongst others.
· Strengthen facility-community collaboration using bidirectional referral to increase uptake of preventive, care and treatment services, improve retention and optimize treatment outcome.
· Participate in the state TWG and engage with the SMOH in health system strengthening to provide prevention, care and treatment services.
· Conduct capacity building (training, workshops, hands-on mentoring) on HTS, PMTCT and other preventive packages for facility and community service providers on routine and need bases.
· Develop SOPs, TORs and job aids to enable standardized service provision at facilities and communities.
· Technical support to the State MOH, KADSACA and the state prevention TWG in development and adaptation of SOPs, protocols, guidelines and other job aids for use across facilities in the state.
· Strengthen implementation of HIV prevention services at the facility and community through conduct of routine data and performance review to identify underperforming SDPs requiring improvement.
· Provide technical support to facilities and CBOs to conduct process review and continuous quality improvement project to address and strengthen HIV prevention services implementation.
· Collaborate with the M&E in ensuring prompt data entry, collation and analysis inform program decisions and direction.
· Lead a MDT responsible for HIV program implementation across 40 sites.
· Collaborate with the HTS team on intensified Paediatric and Adolescent HIV case finding drive through targeted PITC, family index testing (FIT), SNT
· Provide technical assistance to the community testers on Paediatrics & Adolescent HIV case finding drive at hotspots and collaboration with facilities to trace, test and link children of index clients.
· Facilitate 100% linkage of all newly-identified HIV positive child from all testing streams
· Ensure 6-monthly viral load coverage for all eligible CALHIV.
· Collaborate with the Care and Support team to ensure retention in care for all CALHIV
· Ensure achievement of viral suppression with emphasis on U=U for all CALHIV.
· Mentor and support facility service providers on implementation of the various facility- and community-based DSD models for CALHIV
· Ensure ARV optimization for all CALHIV based on age and weight (including DTG and LPV/r).
· Ensure all sexually active ALHIV have access to FP, STI screening and treatment.
· Ensure all pregnant and breastfeeding AGYWLHIV access PMTCT and YFHS services
· Capacity building for all cadre of facility service providers
· Development of SOPs, TORs and job aids to facility for use at facilities.
· Ensure quality data entry and timely availability for reporting and performance review of KPI
· Participate in the State and National TWG meetings
