Working as part of care team providing essential care to elderly with diagnosis such as hypertension, diabetes, arthritis, dementia ,people with disabilities and end of life. Carrying out everyday duties in service users homes with emphasis on promoting and maintaining their well being and independence.

Responsibilities

• Supporting daily hygiene needs of service users by assisting with bathing, dressing, dental care, incontinence and personal grooming, retaining comfort and dignity
• Checking service user vital signs and clinical signs such as temperatures, pulse, weight and blood pressure, oxygen saturation and blood sugar and recording them.
• Laundering items, changing diapers, sheets, underlay and make bed to keep patients' bedroom clean.
• Preparing meals and assisting clients with eating tasks to support client nutritional needs.
• Monitoring clients overall health, mental stimulation and well-being and noting significant changes.
• Oversees medication, prescription usage and administering medication to service users.
• Supporting service users with physical disabilities including morbidity, use and care of aids and personal equipment.

• Followed proper procedures for installation and removal of catheter and IVs.
• Performed routine wound care and dressing changes on schedule.
